In the late 1950s, atherosclerotic lesions in the arteries were intensely scrutinised at autopsy by the Committee on Vascular Lesions, a creative forum that became part of the American Heart Association in 1959. By the early 1980s, concerned efforts brought many medical subspecialties together to quantify treatments of atherosclerosis in the living, by comparing results and standardising on measurements and terms deployed. With improved understandings about the vascular wall, the Committee was able to develop a classification of atherosclerotic lesions.
This work reveals critical understandings about the natural history of plaque, the correlation of vascular lesions and clinical syndromes, and the utility of new imaging methods. Diagnostic imaging, atherosclerosis prevention and treatment will be shaped by these new modalities. Continuous scrutiny by the Committee is culminating in scientific meetings every few years, and is summarised in published statements. The incidence of treated end stage renal disease (ESRD) continues to rise, particularly in the western world. Although renal transplantation is the treatment of choice for ESRD, the decline in the number of cadaveric organs, coupled with the rising demand for transplantation, means that an increasing number of patients will have to depend on dialysis. Vascular or peritoneal access is the Achilles' heel of any dialysis service. This book offers a clear description of the state of the art in providing and perserving a durable and reliable access. It also points the way to dialysis in the 21st century. The contributing authors are drawn from a wide background, with expertise in various aspects of dialysis access, including its history, the technique of placement, anaesthesia, radiology, nursing care and training of vascular access surgeons. There is currently a dearth of books on the subject of dialysis access. The few existing books on vascular access were written in the United States, with none from the UK. This volume is intended for use by transplant surgeons, general surgeons with an interest in vascular access, vascular surgeons, nephrologists, trainees and nurses. Dialysis services are expanding worldwide and this book is a valuable guide for those involved in daily patient care and organization of dialysis services.

Ischaemia-Reperfusion Injury is concerned with the consequences of interrupting and restoring blood flow to tissues. Many common clinical conditions are caused by interruption of blood flow to tissues (eg, ischaemic heart disease, peripheral vascular disease, stroke); blood flow is also interrupted deliberately in many surgical procedures (eg, cardiac surgery, arterial surgery, transplant surgery, limb surgery with tourniquet). In treating such conditions or after performing such operations the aim of the clinician is to restore the blood supply to the ischaemic tissue. Paradoxically, restoration of blood flow to ischaemic tissues can lead to further tissue damage with the potential for severe local and systemic injury. This book focuses on the clinical, pathological and biochemical processes involved in ischaemia-reperfusion injury and gives an overview of the strategies that may be adopted to mitigate or prevent such injury. This book will be of interest to both clinicians and scientists who have to deal with or have interest in this difficult but important problem.
